How to Refer a Patient

To refer a patient to the Emergency Department for immediate treatment, please contact the Director of the Emergency Department on 5022 3419, and provide other relevant information in writing, including patient's history, current medications and test results. This information can be given to the patient or faxed to 5022 3391.

To refer a patient for an elective procedure, please complete appropriate documentation (pre-admission forms) and instruct the patient to take these to the Hospital's Pre-Admission Clinic.
