Community Rehabilitation Centre
The Mildura Base Hospital's Community Rehabilitation Centre opened on the 1st of December, 2000. It was established within the existing Allied Health out-patient department. New therapy programs were added to those already established to enable rehabilitation patients to participate in a variety of individually tailored sessions and to be able to attend for the length of time and the frequency required to promote optimal recovery.
The emphasis has moved to a case managed, goal orientated, multidisciplinary program.
A Community Rehabilitation Centre provides a multidisciplinary rehabilitation service on an outpatient basis. The focus is on involving the clients in an active rehabilitation program with a multidisciplinary rehabilitation team developing care plans and setting realistic goals.
There will be regular case conferences.
