Hospital Admission Risk Program (HARP)
Description of Service
The HARP program is service which supports people in the community to manage their Chronic Illness. The aim of the service is to:
- improve quality of life and enable clients to perform daily activities to the best of their capacity
- reduce avoidable hospital admissions by providing care in the client’s home environment.
- reduce emergency department presentations
Type of services
HARP care streams provided at Mildura Base Hospital include:
- Aged / Complex Care
- Psycho-social needs
- Cardiac and chronic heart failure
Person centred, care coordinated approach directed by individual healthcare needs in consultation with the HARP team, GP’s physicians and other supporting services.
Includes both individual and multidisciplinary group sessions.
Maximum time of service
If the client is stable and has no hospital presentations within the previous 6 month period.
Location of service
Mildura Base Hospital or client’s home environment
Clients can be referred to HARP if they present to MBH Emergency Department with exacerbation of a chronic disease (respiratory, cardiac or diabetic) on 3 or more occasions within a 12 month period.
Referrals are to be forwarded via Argus (For healthcare professionals), mailed or faxed to the Ambulatory Care Department, details are as follows:
