Post Acute Care (PAC)

Description of Service

Post-acute Care (PAC) services aim to assist people discharged from a public hospital, including emergency departments, acute services and sub-acute services, who have been assessed as requiring short-term, community-based supports to assist them to recuperate in the community and to ensure a safe and timely discharge.

Flexible service as indicated by individual needs, May include but not limited to:

• Personal Care
• Home Help
• Respite Care
• Shopping assistance
• Basic wound-care

Referrals

Anyone can make a referral to PAC. In general however, clients are referred prior to or within 30 days of discharge from hospital.

Internal referrals via Mildura Base Hospital internal referral system.
Referrals are to be forwarded via Argus (For healthcare professionals), mailed or faxed to the Ambulatory Care Department, details are as follows:
